JOB SUMMARY

This position provides psychosocial case management services for patients needing assistance. The Care Navigator (LMSW) functions as part of a multidisciplinary health care team working with providers and other clinical staff at Kelsey-Seybold Clinic. The social worker acts as a resource to physicians and staff, as well as a liaison for the patient. Services provided are generally crisis oriented and are geared to assist patients make proper use of the health care system, while improving the outcome of care Social workers will often serve as liaisons to assist patients and collaborate with other health professionals to ensure patient wellness. The Care Navigator (LMSW) will perform comprehensive assessments and treatment of persons whose lives are impacted by cognitive conditions, mental health issues (depression, anxiety, adjustment disorder, Alzheimer’s, Dementia), or a declining capacity to function independently. This position will work closely with Palliative Care facilities. The Care Navigator (LMSW) is responsible for ensuring community resources are available to patients and families. The Care Navigator (LMSW) must maintain case history records and prepare reports. This position will be responsible for making visits to patients at designated hospital (s). The Care Navigator (LCSW) may perform home visits, e-visits, video visits and may travel to multiple clinic locations. Other duties as assigned.

EDUCATION REQUIREMENTS & EXPERIENCE REQUIREMENTS

Education

A.

Master’s degree in social work from an accredited university.

B.

EPIC experience preferred but not required

Experience

A.

Requires a minimum of 3 years of experience in medical social work required.

B.

Palliative Care, Home health or Oncology experience preferred but not required

Licenses

A.

  • Licensed by the Texas Board of Social Work Examiners, (LMSW)

    • A valid Texas driver’s license.

Proven communication and problem-solving skills; Computer literate
