St. Joseph Health Social Worker (MSW) - On Call / Days in Lubbock, Texas

Job Summary: Provides education and support services to patients, families and staff. Provides psychosocial assessment, counseling, crisis intervention, referrals and information to patients and families. As a complex Discharge Planner and a member of the Case Management Department, works collaboratively with the interdisciplinary team to affect positive patient and family outcomes. Serves as resource for less-experienced social workers.

Essential Functions:

  1. Upon request or referral from any member of the healthcare team (i.e. physician, RN) and/or

patient/family, will complete a comprehensive psychosocial assessment to evaluate

patient/family care needs; will provide counseling/crisis intervention/support/education to

patient/family regarding coping with hospitalization, disability, and chronic/terminal illness and

initiate referrals to appropriate resources to meet the needs of the patient/family to ensure

psychosocial and emotional support are addressed.

  1. Serves as a resource to all members of the healthcare team regarding emotional, social, and

psychosocial components of patient illnesses and will ensure understanding and integration of

all aspects of the plan of care.

  1. Adheres to the regulatory requirements for social work intervention/oversight based on state

and federal guidelines (i.e.: ESRD, transplant and rehabilitation patients).

  1. Maintains current knowledge and researches availability of community agencies/resources for

social, emotional or financial assistance. Understands placement intricacies and can interpret

requirements from state, local, and federal agencies to optimize placement of patients in the

most appropriate setting based on patient’s needs.

  1. In collaboration with the RN Case Manager, provides information, education to patient/family on

community resources and options for post-acute care appropriate to the age of the patient

served and will Initiate referrals to community agencies/resources as needed.

  1. Facilitates discharge arrangements in collaboration with patient/family and the healthcare team

ensuring arrangements meet patient’s continuing care needs. Ensures services are arranged

and communicated. As needed, follows up internally to ensure services are provided as

arranged. Identifies and records clinical, psychosocial and financial barriers to a smooth

transition across the healthcare continuum and assists in identifying and facilitating system

improvements.

  1. Utilizes communication, negotiation, and advocacy skills with patient/family, staff, healthcare

team, and community resources. Will maintain collaborative relationship with post-acute

providers and community services.

  1. Serves as a consultant for processing issues such as guardianship, abuse/neglect, power of

attorney, healthcare surrogate, advanced directives, and psychiatric involuntary admission.

Initiates/completes forms required for post-acute placement.

  1. As patient is transferred, communicates pertinent information to next Social Worker and Care

Manager to transition the patient to the next level of care. Documents activities with entries that

are consistent with meeting department/facility requirements, licensure and regulatory

guidelines.

  1. Supports interdisciplinary team conferences and contribute to the planning and review of care

for patients presented as needed.

  1. Adheres to standards, policies and procedures at both the Hospital and departmental level.

Promotes a service orientation in the performance of position duties and responsibilities and in

interactions with patients, hospital staff and visitors.

Minimum Position Qualifications:

Education: Masters degree in Social Work

Experience: Working knowledge/experience in managed care and payor issues

License/Certification: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker in state of Texas (LCSW). Allowed to work with temporary licensure, but must obtain full licensure within 6 months of employment.

Adherence to the National Association of Social Work Code of Ethics

Three years acute care hospital experience
