St. Joseph Health / Covenant Health Director, Inpatient Care Management in Lubbock, Texas

Covenant Health is seeking a full time Director of Care Management with the Case Management Department at Covenant Physician Partners.

Job Summary :

This position is responsible for the overall operation of care coordination functions through oversight of both daily and long term activities of Covenant Health Partners. The Director is accountable for achieving established performance targets through actively engaging multiple disciplines and professional staff. The Director must appropriately manage people, relationships, and processes in order to achieve maximized results. The Director ensures that the department achieves its goals and objectives by engaging internal and external stakeholders in clinical care coordination and outcomes management processes at both CMC and Covenant Women's & Children's. The Director is responsible for the strategic planning, development, administration and operations of Care Coordination. The Director has oversight for all aspects of daily operations to enable long-term and short-term goals to be achieved in a manner that is consistent with the mission, values and strategic goals of the organization. The Director of Care Coordination is expected to have competent leadership and communication skills related to conflict resolution, critical thinking, problem-solving and decision making skills in order to meet patient needs and contribute to a positive care and work environment. The Director will assist the CHP CMO and VP of Finance as needed.

Essential Functions :

Provide leadership to the Care Coordination team who manages a caseload of assigned patients with sensitivity to individual needs. Ensures assigned team receives sufficient guidance and the support necessary to reach departmental goals. Responsible for all identified departmental outcome measures denoted in Covenant Health Partners’ strategy.

  • PLANNING: Performs strategic planning and programmatic development for the Care Coordination Department. Assesses staff's ability to manage and analyze the patient's total care needs across the care-continuum and develops long and short-term goals tomeet those needs. Ensures availability of necessary tools and resources for the efficient management and coordination of patient care. Plans the department’s clinical services and operations. This includes review and utilization of data to assess the need for performance improvement related to all stakeholders. Planning activities include the following:

  • Financial: Assists the CHP VP of Finance and/or CMO in preparation and implementation of budgets. Recommends funding based on thorough knowledge of policies, costs and operation procedures.

  • Personnel: Selects, develops, manages and evaluates direct reports. Mentors and provides opportunities for personal and professional growth for direct reports. Works with CHP Managers of Care Coordination (or other assigned managers) relating to oversight of indirect reports including productivity and conflict resolution.

  • Quality: Utilizes patient focused, continuous improvement techniques to reach key performance targets; seizes opportunities to resolve/improve processes. Ensures inter-departmental collaboration, customer service and monitoring of key systems. Reviews and acts upon key performance management indicators related to the strategic imperatives of the entire system. Monitors services through observation (rounding) and working closely with all assigned managers to ensure quality of care.

  • Plant/Functional Operations: Identifies and resolves departmental equipment needs, equipment volume, supplies/accessories, safety and environment of care. Manages the department in a fiscally responsible manner. Uses resources in an appropriate and cost efficient manner. Identifies, generates and implements cost containment initiatives.

  • Operations: Ensures that the department achieves the vision, goals and objectives of Covenant Health Partners by developing the scope of service, establishing effective relationships with stakeholders, collaborating with other disciplines, strengthening interdisciplinary processes, managing flexible staffing/workload and overseeing care coordination and outcomes management duties. Conducts research as necessary to ensure that departmental practices are current and continue to meet the needs of the organization. Provides support to Managers of Care Coordination to achieve population health goals, contract performance and DSRIP metrics/goals.

  • IMPLEMENTATION OF PLANS AND ACTIONS:Directs, coaches and mentors staff to ensure the achievement of department goals and objectives for each category above. Communicates with and seeks feedback from stakeholders regarding the implementation of department programs and activities. Ability to meet deadlines.

  • MONITORING AND MEASURING DEPARTMENT PLANS:Through measurement systems, processes, and people assesses achievement of departmental goals and objectives and utilizes data to take corrective action as needed. Collaborates with the CHP CMO to establish goals and objectives for the department. Develops strategies and programs to achieve goals, as evidenced by positive quality clinical and administrative outcomes. Monitors achievement of outcomes and goals.

  • LEADERSHIP:Supports the CHP vis

  • ion of care coordination and outcomes management through role modeling behavior. Coaches and influences direct reports and across disciplines to ensure commitment to this vision and its achievement through daily tasks. Emphasizes a collaborative approach and seeks to influence other services by focusing on patient processes.

  • CUSTOMER SERVICE:Establishes and nurtures relationships with multiple internal and external stakeholders to garner satisfactory customer results. The Director is responsible for establishing systems, processes, people, and education that will improve customer satisfaction. Guides, mentors, motivates and engages the staff in assessing, planning, implementing programs and evaluating patient care.

  • PHYSICIAN RELATIONSHIPS:Collaborates with other departments within CHP and/or Covenant Health to develop strategies, formulate plans, and implement interventions based on patient needs. Supports the needs of physicians to provide care for their patients in the most clinically appropriate setting by developing and implementing clinical care coordination and outcomes management practices. The Director will work with other CHP departments to engage physicians as partners to achieve desired results for the success of all parties. Will also collaborate with Covenant Medical Group leadership and/or physicians to achieve success with contract measures and other related processes.

  • PROMOTES STAFF DEVELOPMENT AND EDUCATION:Provides opportunity for formal and informal education to promote ongoing staff competency. Sets standards for performance and ensures timely completion of performance appraisals.

Minimum Position Qualifications:

Education:

  • Bachelor's Degree in Nursing or Healthcare related field or

  • Bachelor's Degree in Social Work

Experience:

  • 2 years management experience in a health care setting.

  • Experience with care coordination/care management

Licenses and Certifications:

  • Current Texas RN License and/or

  • Current LBSW License

Preferred Position Qualifications:

Education : Master's Degree in Nursing, Healthcare Administration, Business Administration, Public Administration.
