The Health Science Center (HSC) is looking for a Community Health Worker II to serve as a member of the Primary Care and Population Health team. Job Description Summary: The Community Health Worker II, under general supervision, recruits patients for the clinical practice, assesses community health needs, and connects patients to social and community support resources. Assists in data collection and program assessment. Provides periodic support to outpatient clinic functions.

Required Education:

  • Associates degree in applicable field or equivalent combination of education and experience.

Required Experience:

  • Four years of related experience in public health, health outreach and education.

  • Ability to speak English and Spanish.

  • Ability to multitask and work cooperatively with others.

Required Special Knowledge, Skills, and Abilities:

  • Knowledge of word processing, spreadsheet, and database applications.

  • Knowledge of health care education.

  • Ability to multi-task and work cooperatively with others.

  • Ability to work with sensitive information and maintain confidentiality.

  • Strong interpersonal and organizational skills.

  • Strong verbal and written communication skills.

  • Required Licenses and Certifications:

  • Driver’s License

Preferred Education:

  • Bachelor’s degree.

Preferred Experience:

  • Experience working with diverse individuals and communities.

  • Six years of related experience in public health, health outreach and education.

Preferred Licenses and Certifications:

  • Community Health Worker Certification

Preferred Special Knowledge, Skills, and Abilities:Experience in a clinical setting, specifically with colonoscopies or women’s health procedures.

Job Responsibilities

  • Participates in patient recruitment, program planning, implementation, and evaluation. Plans, organizes, drives to and participates in community outreach events, health fairs, and workshops.

  • Focuses efforts on diverse populations, utilizing bilingual communication skills

  • Counsels individuals, families, and/or groups and engages in health promotion and disease prevention programs.

  • Coordinates appointments, ensuring that appropriate medical records are available.

  • Manages paperwork to meet patient care needs, including home health documents, nursing home information and specialty consultation referrals.

  • Assists in coordination and communication of laboratory reports.

  • Communicates with patients, serving as a liaison between clinic physicians, staff, patients, and outside providers.

  • Provides assistance to patients seeking financial and other social and/or community support services, including employers, insurance companies, debt management counselors, and patient care coordinators.

  • Supervises Community Health Worker (CHW) students, trainees, and volunteers, both in clinic and community settings.

  • Serves as an instructor to CHW students and trainees in formal, structured programs.

  • Assists clinical support staff in the flow of patients within the clinical environment.

  • Assist in rooming of patients,

  • Assists patients to dress/undress, transporting patients via wheelchair or stretcher, and assisting procedure staff in cleaning and reprocessing medical instrumentation.

  • Provides first aid and basic medical screening services.

  • Collects and analyzes data on clinical services provided to diverse populations and barriers to healthcare access.

  • Participates in research and scholarly activities related to health promotion and disease prevention.

  • Performs other duties as assigned.
