We Are Hiring
Dell Seton Medical Center at the University of Texas in partnership with Dell Medical School at the University of Texas - Austin is recruiting for a Kidney Transplant Social Worker to join the newly developed Kidney and Pancreas Transplant Program!
Strongly prefer the following
Certified Clinical Transplant Social Worker (CCTSW) credential is strongly encouraged.
A minimum of two years of social work experience in a hospital or health care setting.
The Transplant Social Worker's primary responsibility is coordinating the psychosocial needs of transplant candidates, recipients, living donors, and their families. These professionals will work with patients and families in a compassionate, culturally sensitive, and thoughtful way to facilitate continuity of care. Responsibilities will include, but are not limited to:
The psychosocial evaluation of potential living donors and recipients.
Substance abuse evaluation, treatment, referral, and monitoring.
Support groups and newsletters.
Patient care conferences.
Patient and family education.
Referral to community services such as vocational rehabilitation and housing.
Death, dying, and bereavement counseling.
Transplant team building.
Department meetings, including staff and process improvement meetings.
Participation in organ donation awareness initiatives.
Participation with community advocacy groups such as the National Kidney Foundation and the Coalition for Donation.
What You Will Do
Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.
Create plan for care across the continuum, integrating patient/family preferences and values.
Monitor patient care through assessments, evaluations and/or patient records.
Advocate for resources and removal of barriers.
Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
What You Will Need
Licensure / Certification / Registration:
One or more of the following required:
Counselor credentialed from the Texas State Board of Examiners of Professional Counselors obtained prior to hire date or job transfer date.
Social Worker credentialed from the Texas State Board of Social Worker Examiners obtained prior to hire date or job transfer date.
- High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.
