Baylor Scott & White Health PRN Diabetes Peer in Dallas, Texas

The Diabetes Peer will have a deep understanding of their community and its cultural mediator between their church/faith community, Dallas Health Wellness Institute (DHWI) and the Baylor Scott & White Health (BSWH). The Diabetes Peer will work closely with the Community Health and Diabetes Educators to provide basic diabetes and health education, informal counseling and self-management support that is culturally appropriate to their specific setting. The Diabetes Peer will be required to successfully complete State of Texas approved Community Health Worker (CHW) training program and obtain the Texas CHW certification within 6 months of hire. Responsibilities of the Diabetes Peer role includes, is not limited to, the following:

  • Assists with the tailoring of a faith-based diabetes educational program that is relevant to the unique beliefs of the Diabetes Peer's faith community.

  • Promote the provision of culturally competent care by educating DHWI staff about beliefs and practices within their faith community.

  • Recruit fellow parishioners who have diabetes to participate in a diabetes education program.

  • Collaborates with Community Health and Diabetes Educators in supporting prescribed therapies.

  • Promote the provision of culturally competent care by educating DHWI staff and others within the health care system about beliefs and practices held by their faith community.
